Declaration, authorisation and data privacy consent

1. I acknowledge and agree to the collection, use and disclosure of my personal data which has been provided for the purposes of procuring insurance products & services as per the terms in DirectAsia's Personal Data Protection Statement.
2. No rider has had their licence suspended or cancelled in the last 5 years.
3. No rider has been refused insurance or had it terminated in the last 3 years.
4. The motorcycle is registered in Singapore and any modifications it may have are LTA compliant.
5. The motorcycle is not used for commercial, leasing, rental and/or hire and reward purpose.
6. The policy covers you for food and/or parcel delivery if Deliveries usage is declared and selected.
7. You had read and understood fully the coverage and terms/conditions of the policy you are purchasing including any optional benefits taken.

Please note, we are also required by law to tell you that before we provide cover, you must fully and faithfully tell us everything you know or could reasonably be expected to know that is relevant to our decision to give you the insurance cover, otherwise you may receive no benefit from your policy or we may cancel your policy and treat it as never having existed.
