Car and Motorcycle Cancellation Request Form

Sold or scrapped your car or motorcycle?

Request to cancel your policy by completing the form and providing the necessary supporting documents below. Cancellation cannot be more than 30 days before or after today's date and policy coverage will cease 1 day after the vehicle de-registration or ownership transfer date.

Cancellation date refers to the date 1 day after you have de-registered the vehicle under your name with LTA or which you have transferred the ownership of the vehicle to the new owner as you will still require coverage on the date of de-registration/transfer of vehicle ownership. For example, if the date of transfer of vehicle ownership is 20th January, the cancellation date will be 21st January.

Refund details

Please select a payment mode and provide your details on how you would like to receive your refund. All refunds will be subjected to a cancellation fee of $90 (car insurance) / $53.50 (motorcycle insurance) (include of GST). No refund will be given if there is an accident reported and/or claim made under your insurance policy. We reserve the right to collect any outstanding premium from you if you have opted for our instalment payment plan.

Declaration, authorisation and data privacy consent

  1. I acknowledge and agree to the collection, use and disclosure of my personal data which has been provided for the purposes of procuring insurance products & services as per the terms in DirectAsia's Personal Data Protection Statement.
  2. I, the Policyowner of the above policy hereby request for cancellation of the above policy and accept that Direct Asia Singapore Pte Ltd. shall be fully and completely discharged of any liability upon the cancellation of the policy.
  3. I agree that the original Certificate of Insurance remains the property of Direct Asia Insurance (Singapore) Pte Ltd. and following cancellation or replacement, it will have no value and that it cannot be used as proof of cover.
  4. I declare that I have not amended, transferred, or traded the Certificate to any third party or used it in any formal capacity as proof of cover or value.
  5. I hereby acknowledge and confirm that there are no incidents which will give rise to a claim during the current period of insurance. In case of any claim (including a pending or reserve claim) under the Policy, Direct Asia Singapore Pte Ltd reserves its rights to recover the refunded amount and / or repudiate the Policy against all claims.
  6. I am aware that I will not receive any refund of premium if on or before cancellation of this Policy:
    1. Any claim has been made or expected to arise under this policy;
    2. The premium has not been paid
  7. I am aware that my refund will be pro-rated less a cancellation fee of $90 (car insurance)/ $53.50 (motorcycle insurance) (inclusive of GST).
