Notify us as soon as possible and submit your claim within 30 days of the accident with the following original documents:

	Completed claim form
	Medical report and other medical documents
	Medical bills
	Police report (only needed if an injury/death resulted from a traffic accident, crime or malicious act)
	Documents regarding the deceased. These include death certificate, autopsy report, coroner's findings
